October 22nd Update: H1N1 Vaccine Clinic Update

  • Permission forms for students to receive the H1N1 vaccine will be mailed home once received from the NDDH (Northeast District Department of Health)
  • Forms need to be returned promptly in order to insure an accurate count
  • Clinics in the schools will be starting mid to late November

September 24th Update

Woodstock Academy and the Northeast District Department of Health (NEDH), will be offering the H1N1 (Swine Flu) vaccine to our students in grades 9 – 12. As children spend a great deal of time in school, the vaccine is being recommended by NEDH.  It is not a mandatory vaccine at this time. The recommended age for the vaccination is 6 months to 24 years.

  • When we receive the permission slips from the Northeast District Department of Health, we will distribute these slips to our students to bring home for you to sign.
  •  Each parent/guardian must sign the permission slip indicating whether or not you grant permission to have your child receive the vaccination.
  • It is crucial that the signed permission slips are returned to the Health Office so that there will be the correct number of doses available at the clinic.
  •  If we do not receive a signed permission form for your child(ren), we will not be able to give  the vaccination to your child(ren).
